All records of teaching evaluations, observed classes, and other matters related to teaching are located in [room and building], the central office of the Intensive English Program. A file is also maintained on each graduate teaching assistant in the Department of Foreign Languages in [building name]. These records contain admission information, contracts, and copies of student evaluations of teachers. Full-time teachers are required to submit an Academic Productivity Report at the end of each calendar year. This report is submitted to the Department Chair for review by a committee and substantiating documents are signed into the instructor's permanent file in the presence of the departmental secretary. Items cannot be withdrawn from this file. Teachers have access to their own records upon request. However, all records are confidential and cannot be accessed by others. In addition, all personnel, teaching, and supervisory matters which pertain to the individual rather than to the program as a whole should be considered confidential by the parties involved. Failure to comply with these confidentiality policies may result in disciplinary procedures within the department or the University. |
